Majid Bin Said the first Sultan in Zanzibar gave it to Two Arabs who used it as a Prison for rebellious slaves prior to shipping them abroad or selling them at slave market in Stone town Zanzibar. In October 1891 the British Lloyd Mathews purchased from Arab with intention of building a prison on it. The prison was to have housed violent and recidivist criminals from the part of African mainland which was under the jurisdiction of Zanzibar.

Despite the prison buildings being completed in 1894, causing the island to be come known commonly as the PRISON ISLAND. In 1923 the old prison was converted into facility's hospital and the island was officially renamed 'Quarantine Island' and all the Quarantine cases would be taken from th ships and monitored for one or two weeks before being allowed to progress with their journey.

The main disease that was monitored was Yellow Fever. In 1919 the British governor of Seychelles sent a gift of four Aldabra giant tortoise to Changuu from the Island of Aldabra. From now the Quarantine station converted to guest house, European Bungalow turned into Restaurant and the island owned by the Government of Zanzibar.
